Our Current Initiative: Pilot Project in Vieques

At NUR Salud Comunitaria, our work is rooted in taking action where it’s needed most, and our pilot project in Vieques is a reflection of that commitment. Vieques, a small island municipality off the coast of Puerto Rico, has long faced serious challenges in accessing healthcare—from the closure of its only hospital after Hurricane María in 2017 to ongoing gaps in the availability of essential medical services.

Our pilot project aims to bring healthcare directly to the community, focusing on providing accessible, community-centered services for residents who often face long distances, transportation barriers, and limited medical infrastructure.

What We’re Doing

In-Home Health Services

We conduct home visits to provide basic health checkups, support for managing chronic illnesses, and medical follow-up for individuals who face difficulties accessing healthcare.

Preventive Health Screenings

We offer on-site health screenings for hypertension, diabetes, mental health, and other key indicators, aiming to identify health issues early and connect residents with the care they need.

Health Ambassadors

We employ and train local residents to serve as health ambassadors, equipping them with knowledge and tools to support their neighbors and act as a bridge between the community and healthcare providers.

Health Education Workshops

We organize community workshops focused on disease prevention, mental health awareness, nutrition, and general wellness, ensuring residents have the knowledge they need to prioritize their health.

Patient Navigation Support

We help individuals schedule medical appointments, coordinate referrals, and overcome barriers to accessing healthcare—ensuring that everyone receives the care they deserve.

Why this matters?

In Vieques, delayed or inaccessible medical care can mean the difference between life and death. The lack of medical facilities and the long, costly trips to the main island have made access to healthcare a daily challenge for residents.

Our project is designed to eliminate those barriers, ensuring that medical care reaches every person, regardless of their circumstances.

Looking Ahead

Our pilot project is just the beginning. Our goal is to expand our services to more communities in Vieques and use this model to inform and inspire similar initiatives across Puerto Rico.

Through this work, we aim to create a system where medical care isn’t something people have to chase, but something that reaches them directly.

Together, we are building a healthier and more resilient future—one home, one family, and one community at a time.
